On this page you will find the forms you need to complete in order to prepare for your arrival at Warren Wilson College. These forms must be submitted 14 days after depositing, except for the health form and tax forms which must be received by August 1st. Please scroll all the way to the bottom of this page for more exciting forms to fill out.

The health form must be printed out and submitted to your physician and then mailed to the college by August 1st. If you have any questions about the form please email the Health Center at: healthcenter@warren-wilson.edu

The tax forms also must be printed out and mailed by August 1st.
